Application for Associate Membership
Associate Members of the society will receive a CD-ROM fitting guide to Orthokeratology, as well as being able to participate in future training courses and meetings organised by the society.A member who then wishes to become accredited in the technique needs to submit five case records for peer review. Upon becoming an accredited member of the society, the practitioner will be promoted as an accredited orthokeratologist. In applying for membership I confirm that
I will not perform the technique and remain a member of the society unless I have satisfied these requirements. My name and details will appear on this list unless I specifically request that it is removed. Please print out this form, fill it in and send it with a cheque for £100.00 to The Secretary
